Welcome and thank you for the opportunity to serve your healthcare needs.

How should you prepare for your first visit?

Fill out and bring with you all of the appropriate paperwork.  It is nice to allow time to thoroughly answer the questions in the health history, so we can provide the most comprehensive care.

Please arrive on time, all effort will be made to be respectful of your time by keeping on schedule.

What should I bring?

1. your completed intake form
2. ALL supplements and medications you are currently taking regularly.
3. Any lab work done within the last year, or a list of medical providers so that we can request records.
4. Insurance card (if you plan to use insurance)



How can I find you, I don't see your sign?

I practice within another clinic named LACROUTE NATURAL MEDICINE, on the corner of NW 9th and Glisan in the Pearl District.

Is there Parking?

You can find 90 minute, 2, and 3 hour metered parking within one block of the clinic.  Please plan your parking according to your visit.  New patient visits may last longer than 90 minutes, so I recommend parking in a 3 hour area. 

I am receiving Hellerwork sessions, what should I wear?

Bodywork clients should wear comfortable clothing for postural and movement assessment.  During Hellerwork sessions, it is best if women wear a sports bra or tank top with loose exercise shorts and if men wear loose fitting gym shorts. This allows for flexibility of working on and off the table.

How can I pay for my visits?

You may pay with cash, check or credit card.  Bodywork sessions may also be purchased on-line

Do you sell Gift Certificates?

Absolutely! You can buy them directly from me or I can mail them to you.

I am trying to schedule on-line with "Schedulicity", and I wish there was a little more flexibility to the time displayed. Can I schedule directly with you? 

Schedulicity is great, and cuts down on phone-tag, but the timing options are not always perfect.  You can always contact me with a day and time that works best for you and I will do my best to accomodate.

Do you do home or on-site visits?

I am open to traveling, and have a massage table that is portable.  I charge an additional $30 to travel.   I may require an initial visit in person before traveling to a patient's home.

Insurance:

I am currently in the process of becoming a provider on Insurance panels.  At this time, I am only a massage provider through United Healthcare. I can also accept Auto Insurance.

If your insurance allows billing for "out of network" providers, then I will bill your insurance on your behalf.  It is your responsibility to know your insurance coverage.  You may download a form to check your insurance benefits for Naturopathic Medicine or Massage below.

I will provide a receipt with insurance billing codes for you to submit to your insurance for reimbursement if I am not a preferred provider.
